[lttng-dev] Fixes about UST wrapper ip context

Hi,

I pushed fixes into lttng-ust master, stable-2.6 and stable-2.5
branches which ensure that the "ip" context for malloc, mutex,
cyg_profile, dl instrumentation are the instruction pointer of
the application calling into the "trampolines" rather than the
address within the trampolines per se.

In the master branch, I removed the recently added "caller" fields
from the malloc events, since it now duplicates the "ip" context.
